PGIMS doctors remove large Gossypiboma from 55-yr-old man

PNS

The Medical Gastroenterology department of PGIMS, Rohtak, has achieved a rare feat of successful endoscopic removal of large mop (Gossypiboma) which was stuck up in a patient’s stomach and duodenum.

The head of the department, Dr Parveen Malhotra performed this procedure along with his team comprising of Dr Ajay, Jaikishan, Naresh and Sandeep.

He informed that this 55-year-old male patient had undergone gall bladder removal five months back at some private hospital. After few days of operation he developed right sided pain abdomen, persisted vomiting and loss of appetite. He was investigated for this at various private hospitals but got no relief. later on he was referred to PGIMS, Rohtak, and was admitted in surgical ward under Dr RS Dahiya, with suspicion of gastric outlet obstruction on basis of Barium meal and ultrasonogram. When endoscopy was performed it revealed a large mop Gossypiboma stuck at pyloric antrum of stomach and duodenum. As this mop was stuck up and had large size, it was very difficult procedure and took around one hour. Moreover, any trivial mistake could have led to perforation of stomach or duodenum, he added.

Regarding the origin of this mop, Dr Malhotra elaborated that this mop would have been left by mistake during gall bladder removal and it is called Gossypiboma.

In very few patients this Gossypiboma erodes stomach or duodenum and gets migrated into them, same happened in this case. It is probably first in India where such large mop was removed successfully by endoscope and hence, patient was saved from laparotomy. Now patient is asymptomatic, accepting feeds properly and ready for discharge.
