A patient who is a resident of Bhopal was suffering from OSA, a sleep related breathing disorder. After sleep he had continuous snoring causing disturbed sleep and temporary cessation of breathing.
Due to break in the sleeping cycle it was very difficult for him to sleep whole night and he was restless the whole next day and it increases the day time sleeping to complete his sleep. Day time restlessness was very disturbing for him and he was not able to concentrate on his work and his social and family life was also getting affected.
After consulting several doctors, he came to AIIMS Bhopal and met Dr Anshul Rai who advised him to go for sleep study, and after investigations it was decided to go for jaw advancement surgery. Dr Abhishek Goyal confirmed the diagnosis of OSA after the sleep study.
In the sleep study, the Apnea- Hypopnea score was very high. At first the oral appliance was given to the patient, for wearing it at the night to improve his sleep but the oral appliance did not work well. After the cefalometric analysis the pateint's lower jaw was found to be deformed and retruded causing OSA.
Dr Anshul Rai decided to go for the orthognathic surgery of the lower jaw to bring it forward by Bilateral sagittal split osteotomy of the mandible. This surgery was very complicated and required expertise; it is performed only by few doctors in whole of MP. The surgery is very expensive too.
The lower jaw advancement surgery increases the breathing space, reduces snoring, improves sleep and patient can be relieved from OSA which indirectly improves the quality of life of the patient.
The deformed jaw was also corrected that improves the facial profile of the patient and improves his confidence. After two months of surgery sleep study was again carried out and the apnea hypopnea index was reduced via 70% and thepateint'smany problems were sorted out.
After the teeth alignment, sleep study will be carried out again to see the progress. The patient is now happy and is able to have good, unobstructed sleep after many years.
